9(C) Seaman's Visa

Applicants who will come to the Philippines to join a vessel are required to fill out the  Visa Application Form and submit the requirements to parispe.consular@dfa.gov.ph as one (1) PDF file. 

MANDATORY REQUIREMENTS

1. Original and photocopy of passport valid for at least six (6) months beyond the intended  period of stay in the Philippines 

2. Original and photocopy of valid Seaman’s Book with latest log entry 

3. One (1) passport-size photograph with white background, taken within the last six (6)  months from the date of application

4. Sponsorship letter from the shipping company indicating the name and designation of the  seaman, name of the vessel to be joined, and the place where the vessel is docked in the  Philippines. 

5. Endorsement letter from Philippine-based agent acknowledging the incoming seaman’s  assignment aboard the vessel, duly notarized by a Philippine notary public. 

6. Proof of financial resources (e. g. bank account statement or pay slip)  

7. Itinerary of travel/tentative booking showing the departure from France and date of arrival in  the Philippines
