Self-assessment and Symptom Monitoring Requirement

In reference to CDC and DOH issuances (https://www.cdc.gov/ and https://doh.gov.ph/ ), self-assessment and symptom monitoring shall be done by all before leaving their houses. On this note, all stakeholders are expected to do self-assessment or self-screening before coming to school. Parents must screen their children before sending them to school for their in-person classes.


a. Those who are experiencing any of the COVID 19 symptoms or are not feeling well are obligated to take necessary steps to ensure the health and safety of others, including:


  • Staying home and not coming to campus;

  • Self-isolating and avoiding social gatherings and contact with other people;

  • Informing immediate head/supervisor (for employees) or teacher/adviser (for pupils and students);

  • Consulting with healthcare providers and following their health advice (testing and treatment);

  • Informing Local Government Units (LGU) or Barangay Health Office if tested positive for COVID-19; and,

  • Visiting DOH website for guides and additional information.


b. To supplement the self-assessment, safety and security personnel assigned at the gates shall conduct health status screening for all participants of the pilot testing before they are allowed entry to the school premises/buildings. All school personnel, parents, students and guests entering the school shall:


  • Have their temperature checked. An individual whose temperature is greater than 37.5o C (even after a 5-minute rest) shall not be allowed entry and he/she shall be brought to the designated isolation area.

  • Accomplish daily health symptoms questionnaire and submit it to the security personnel or to the assigned clinic staff. An individual with a response(s) needing further evaluation by the clinic staff shall not be allowed entry, and he/she shall be brought to the designated isolation area for a more thorough assessment.
