A Field Trip form must be completed for any over night trip or trip outside the Denver/Metro area. This request form must be completed and submitted to the Athletics Director a minimum 30 days prior to the planned trip when possible.
Information needed when requesting a field trip include:
Mode of transportation, Location of event (including address), Location of hotel (Overnight), Nearest Hospital,
Chaperone Names, Names of Athletes attending.
You may upload a roster and it is a good idea to have a specific itinerary to upload as well.
Depending on the trip, there are parent permission forms that may need to be signed.
