GRADUATING SENIORS- EXIT PROCESS
Exceeding Age Eligibility or Granted a Diploma
Students are no longer eligible for special education and are excited when they graduate from high school with a regular high school diploma or reach the age of22 years (E.C. § 56026(c)(2)-(4)). It is not necessary to conduct an assessment for students exiting under these circumstances, but the IEP team must provide the student with a summary of his or her academic achievement and functional performance including recommendations on how to assist the student in meeting his or her post-secondary goals (E.C. § 56381(c)(1)-(2)).
CASE MANAGER RESPONSIBILITIES
The following protocol will be used when transitioning graduating seniors from the District's special education program/services.
1. Case Manager to create a New Meeting for the student in SEIS (date/time).
a. No Meeting Notice is Required
b. No Administrator, Parent or General Education Teacher is required
2. New Meeting Purpose. select Exit Summary.
3. The Case Manager will complete the forms in SEIS. This is used to document that notice has been provided to a student who is being exited from special education due to graduating with a high school diploma or reaching the maximum age for special education eligibility. Note: The Exit Date and Reason when applied to the forms will be entered automatically onto the MIS Summary page after the meeting has been held.
4. The Case Manager:
• Completes the Exit Summary form
• Meets and explains to the student the Exit Summary form
• Obtains student signature anywhere on the form
• Gives copy of the Exit Summary to the student
