Transition Teachers Offer the Following Services:
Transition SAI
Student Transition workshop. GE students and students with 504 regularly invited.
Agency linkage- Connect students to Regional Center (RC), Department of Rehab (DOR), and Apprenticeships. In our first year, over 30 students across the state have connected with the DOR and are getting paid work experience. Now we have partnered with JobCorp and are holding informational sessions every month.
Personalized career assessments/guidance/counseling using multiple modalities.
Transition Teachers are Responsible for the Following:
Writing the Individualized Transition Plan (ITP) pages of the IEP and related pages ( PLP's).
Writing, tracking, and documenting progress on Transition goals/services.
Attending IEP meetings and reporting out on the areas listed above.
Assessing and writing a standalone Transition report for all Triennial IEP’s.
Teaching Transition SAI several times a week.
Indicator 13 & 14 of the State Performance Plan (SPP) as mandated by IDEA 2004.
Adult Transition Program Teachers and Job Coaches Provide and are Responsible for the Following:
Case manage ATP students.
Provide SAI and Transition SAI.
All ATP/Job coaches are CPI (Crisis Prevention Institute) trained.
Job Coaches teach sessions on topics such as:
Pre-employment skills
Career Interest Surveys
Job Application and Resume
Interview Skills
Appropriate work conversations
Job Coaching and Job Development
Agency linkage.
Community-based instruction (CBI).
Collaborate with RC and DOR for a smooth transition to their agencies once students age out.
