Discounts are available for staff at the discretion of the GM, based on consistent performance against KPIs and evidence of living the philosophy. Access to discounts will be considered after successful completion of probation, and is only available to full-time or part time staff contracted for 3 or more days per week. In the case of complex treatments, informed consent will be managed through the standard provision of a written treatment plan provided to the patient. Staff will be liable for all relevant third party costs incurred by Bite or TSD, before any discounts are applied. These costs include:
Lab costs
Whitening consumables
Commission for associates (this can be negotiated directly with the associate)
Third party treatment such as specialist fees, hospital fees etc.
After paying for these costs, the maximum treatment discounts will be:
Staff - Discounted to $100 per hour paid by a combination of health fund and out of pocket.
Immediate family (spouse, defacto, parent, child, sibling) - 25% off out of pocket (see Terms and Conditions below)
Other family (grandparents, aunts, uncles) - 10% off out of pocket
Terms and conditions:
Qualifying conditions may be waived by the GM if treatment is considered to be a valuable component of the staff member's Professional Development.
In exceptional circumstances, and after discussion with the GM. some standard terms and conditions may be waived, or additional terms and conditions may be applied.
The entire fee must be paid in advance unless payment terms are agreed.
Treatment will be undertaken at a time that suits the practice, and when the staff member is not "on the clock".
All appointment times must be agreed in advance with the GM, and will be subject to change without notice depending on patient demands.
The discounted amount will be capped at $3,000 per annum per staff member
Two plans cannot be active concurrently for (or on behalf of) one staff member, regardless of the value.
Discounts are a privilege, and Bite/TSD reserves the right to withhold or cease treatment if the staff member leaves the business or enters a Performance Improvement Plan. In such instances, the practice will release records to the patient to enable treatment elsewhere and refund any unused funds.
Discounts cannot be applied to promotional offers that are already discounted below the relevant staff benefit.
In all cases, this form must be completed by all parties prior to treatment commencing.
Failure to comply with these Terms and Conditions will result in the treatment being undertaken at full cost.
