All incidents that occur on the practice, or as a direct result of treatment, are to be lodged via the incident register form Incident categories include:
Needlestick injury (Dentists can complete a referral to S&N. Pad in GM's office)
Cut
Bodily fluid exposure
Adverse reaction
Other WHS incident
Patient complaint
Staff complaint
Drills
Other
All staff members are responsible. If you suffer an injury, witness an incident or receive a complaint, you should lodge it. If you are unsure, speak with your line manager or the GM.
The GM, in collaboration with the Practice management team, is responsible for managing all incidents. Resolutions will be tracked in the Incident Register (responses) Sheet. This response sheet can only be edited by the GM, to ensure that all incidents are addressed for quality and safety purposes.
