Go to ‘patients’ tab
Select ‘+1’ symbol
Enter details accordingly e.g. first & last name, mobile number and email.
Ensure to include who they were referred by e.g. DBoss, another patient etc and select which dentist they will be seeing
DBoss patients will always have their first appointment booked under Emily
Include any other information e.g. address, DOB etc
Search by patients last name under the patients tab
Select the ‘Medical’ tab
Select update medical list
Fill according to the medical update sheet completed by patient
Scan and upload the completed form under the ‘Contacts’ tab, titled “Medical History 1” and “Medical History 2” for new patients, or “Med History Update” for existing patients

Select the ‘charts’ tab
In the charts tab select the image of an eye in the images section
This will open EXPRO
Once in EXPRO select file (top left corner) -> import image -> double click what image you want to import -> select ‘import’ button -> repeat steps for all images that need to be imported
Open Inteleviewer from the computer’s desktop
Select the login portal depending on the company that did the scan (referral may be provided by patient), all logins will be saved in DFS
We typically use Citiscan the most
Enter the patient’s full name using the format LAST,FIRST
If a pop-up occurs where you ‘do not have access’ to this patient, use the ‘Break Glass’ feature
Enter the patient’s full name with the same format as before, and their DOB using the format YEARMONTHDATE (no spaces) and select ‘Break Glass’ button
This will trigger a patient’s file and you can access their scans to download
Click the file button in the left hand corner, and ‘export image’ to a location its easily accessible
Upload to patient’s file in Exact as described above

Written Medical Consent must be obtained by the patient signing a Medical Consent document using the iPad (Bite) or signing a printed consent form (TSD)
How: In Exact - Patient chart - Consents tab (bottom left). Click "Sign consents". Choose appropriate template - send to Clinipad - Give iPad to patient to sign. Or print and scan back in once signed.
When: Prior to the procedure, so preferably while the appointment is being made at Reception, but if not possible then when they arrive for the appointment
Who: Reception staff are responsible for securing signed consent and filing it on the patient record.
