LOA APPLICATION

A scholar may file an official Leave of Absence for a maximum period of one whole academic year. After which, the scholar shall be obliged to return, otherwise his/her scholarship will be immediately terminated due to breach of contract.
Requirements:
Duly accomplished LOA Application Form
Complete Grades (refer to NOTES)
LOA Certification (refer to NOTES)
Statement of Financial Assistance (refer to NOTES)
Medical Certificate and other documents if applicable
Intent Letter (refer to NOTES)
NOTES:
Complete grades must be personally requested from the Office of Admission and Aid (OAA).
LOA Certificate must be requested from the Office of the University Registrar (OUR).
Statement of Financial Assistance must be personally requested from a DOST-OAA staff.
Intent letter must have the following inside address:
