 & Study Away

The Doctoral Programs in Higher Education at Azusa Pacific University offer unique experiential learning as part of both the EdD and PhD programs. APU Center for Career & Community Engaged Learning (CCEL) is pleased to partner with APU Higher Ed regarding a portion of the operating process necessary for these domestic and international experiential learning programs. 

HIGHER ED 2023-2024 PROGRAMS

The following programs are tentatively moving forward for the 2023-2024 academic year:

Participants of these programs can refer to the information on this site regarding any necessary forms/items to submit in preparation for participation.

HAWAII ACTION ITEMS

Spring 2024 Hawaii program participants: submit each item listed below. As CCEL receives participant information this site will be updated to include a chart to clarify which items are completed and which are remaining. Submit items individually or as multiple attachments to one email. Email studyaway@apu.edu with any questions. NOTE: The changing nature of travel requirements due to the global pandemic may affect changes to travel documentation required by universities. All updates will be posted to this site & communicated to participants.  

1.) Travel ID Copy, Due: Friday, March 15, 2024

A. Scan or take a quality/clear picture of the drivers license or other government issued photo ID you will use for travel.  


B. Email the scan or copy to studyaway@apu.edu and include the following: 

i.) your first & last name, 

ii.) your APU ID number (for students and APU faculty/staff) 

iii.) your program name "PhD Hawaii Spring 2024" and 

iv.) the attached scan or copy of the ID.

2.) PHI Form (Personal Health Information), Due: Friday, March 15, 2024


A. Access the form using the following link: https://formstack.apu.edu/forms/personal_health_information_form - Upon completion, you will receive an emailed PDF of your responses. It is that PDF that is needed to complete this item.

B. Retrieve the PDF of your response that will be automatically emailed to you after clicking "submit form". The PDF will be sent to the same email you use in the form.

C. Email the PDF of your form responses to studyaway@apu.edu  

3.) Student Agreements Form (with waiver), Due: Friday, March 15, 2024

This form is processed through Dropbox Sign (formerly HelloSign). Upon completion, you will receive an emailed PDF of your responses. It is the PDF submission that is needed to complete this item.

A. Access the appropriate link: Clicking on the link will send a confirmation request to your email account.

B. Email Confirmation: Open the Dropbox Sign email & click "Review Document"

C. Complete the form with all needed information, signatures, initials, and dates. Click on "Complete".

D. After 5-10 minutes an email will automatically be generated to your email. It will contain a PDF attachment of the form you just completed. Email (no upload/canvas option) the PDF of your form responses to studyaway@apu.edu 

CHECKING ON SUBMISSIONS

The Higher Ed Department has asked for you to email Dr. Lunceford if you have any questions on what has been submitted and what is still needed.

SOUTH AFRICA ACTION ITEMS

Summer 2024 South Africa program participants: submit each item listed below. Submit items individually or as multiple attachments to one email. NOTE: The changing nature of travel requirements due to the global pandemic may affect changes to travel documentation required by universities. All updates will be posted to this site & communicated to participants.

NOTE: The Higher Ed Department will be forwarding to Study Away all submitted passport copies. Therefore, if you have already submitted your passport copy to them, then you may not need to do this step. Check with Dr. Laurie Schreiner if you have any questions. 

A. Scan or take a quality/clear picture of your passport.


B. Email the scan or copy to studyaway@apu.edu and include the following:

i.) your first & last name

ii.) your APU ID number (for students and APU faculty/staff) 

iii.) your program name "PhD South Africa Summer 2024

iv.) The attached scan or copy of the passport.

INTERNATIONAL INSURANCE

The following information is for APU Study Away programs that include a required international travel component. It is only the International program participants (registered students and APU faculty/staff) that receive access to the below insurance as part of the required program charges and after/if all program requirements are completed and maintained (forms, registration, payments, policy compliance, etc).

Review the 3 updated documents to be familiar with the plan. 

Study Away will be submitting student and faculty/staff participants' name and ID number (once all program action item requirements are complete) so that you are covered through the above policy. If you need to use the insurance while abroad, you must save documentation of every transaction (including but not limited to: doctor visits, hospital visits, pharmacy receipts, etc). All documentation will be required in order to submit a claim form for any type of possible reimbursement. Claim forms are to be submitted soon after any incident.
