A new Immunization Certificate** showing your child has received a secod Meningococcal vaccination before enrollment is required. If your child has an exemption form on file, these do not expire.
**Last year, we ran into doctor's offices' computer charting systems, not 'talking' with the state immunization registry, which made it look like students were missing vaccines when they did get them. If a reminder is sent out that your child is missing a vaccine but you are certain they got it, please contact your doctor's office; they will have to manually enter it into the State Registry. Once this is done, have them fax the new Immunization Certificate to the school.

You will get the final copy of your Immunization Certificate or Exemption form prior to graduation. DO NOT LOSE THIS--believe me, having this document will be very helpful for college and possibly employment.
The school will keep a copy on file for 5 years past your graduation date incase you misplace it. You may also get a copy from your doctor's office.

Locate a clinic/doctor.
Know if you have health insurance.
Call the clinic or doctor's office.
Make an appointment time that works for you.
Come prepared and bring with you important documents (drivers license, insurance card).
Come early to your appointment (10-15 minutes is standard). Some clinics will not see you if you are too late for your appointment.
Sign-up for the online health portal (if offered). This is an easy way to reference your medical information if you have a hard time remembering. You can also message your health care team questions and possibly request appointments.
