No new forms needed. Unless forms are missing from Kindergarten or your child is enrolling for the first time (see KDG list).
It is still recommended you schedule a yearly well child visit.
It is good practice to turn in a new signed Immunization Certificate every time your child receives a new shot.
If your child was up to date on their shots last year, they are good again this year.
If your child has an exemption form on file, these do not expire.
Free Health Screen Offered
Free Health Screen Offered
Dental Screenings with fluoride and sealant applications are offered for all 5th graders, free of charge by Public Health.
A consent form must be completed for your child to receive this screening/application.
More information will be sent out closer to the date, which is TBD at this time.
Hearing Screenings are completed each fall on all 5th graders by the AEA.
Students are automatically screened unless the parent/guardian submits a note asking the student to be excused from the screening.
