No new forms needed. Unless forms are missing from Kindergarten or your child is enrolling for the first time (see KDG list) OR if your child is participating in sports--then a new Athletic Physical is required.
It is still recommended you schedule a yearly well child visit.
It is good practice to turn in a new signed Immunization Certificate every time your child receives a new shot.
If your child was up to date on their shots last year, they are good again this year.
If your child has an exemption form on file, these do not expire.
