Kindergarten/TK

• AGWSR District requires, at minimum, one physical record turned in upon enrollment to school

• Any physical after age 4 qualifies. 

• A form (NOT note) provided by your doctor’s office works as well

• Very important: A note from the provider stating your child is ok to participate in school does not satisfy this requirement.

• Must be completed no earlier than 1 year prior and no later than 6 months after enrollment

• Can be done by an eye doctor or family physician.

• No specific form, but may use the IHHS (IDPH) form found by clicking the purple bar above. If this is marked on the physical form, no need for additional forms.

• Free vision screenings will be available through KidSight in the fall for ages 6mo-Kdg (date TBD).

• Must be completed no earlier than age 3 and no later than 4 months after enrollment

• Can be provided by a licensed dentist, dental hygienist, physician, physician assistant, registered nurse, or ARNP

• Must use IHHS Certificate of Dental Screening form

• All children enrolling Kindergarten are required to have at least one lead test to be in compliance with IAC 641 Chapter 67. 

• Your health provider can indicate this testing completion on the physical form. 

• Any lead testing done in your child’s life-time counts towards this. 

• There is no specific form.

• Fully completed and up-to-date immunizations are required for continuous enrollment and attendance

• Your health provider can provide you with a copy of your child’s immunization record. It must be signed by a Health Care Provider. Screenshots of electronic medical records do not satisfy this requirement.

• Religious or Medical Exemption forms will also be accepted

• One of these records is required to enroll in public school pursuant to Iowa Dept. of Public Health 641, Chap 7

Dental Screenings (no treatments) will be completed, before Winter Break, on all Kdg students who do not have an IHHS Dental Certificate on file required for enrollment.

Students without forms on file will automatically be screened unless the parent/guardian submits a note asking to exclude the student.

Hearing Screenings are completed each fall on all Kdg's by the AEA. 

Students are automatically screened unless the parent/guardian submits a note asking the student to be excused from the screening.

Screening Dates for 2025: 

Thursday, October 2nd- 9am

Re-screening/catching those gone on the 2nd:

 Thursday October 16th- 9am.

Offered yearly, in the fall, by Parkesburg Lions Club.

A consent form must be turned in. Students without consent forms will not be screened.

This does satisfy the state requirement needed for enrollment.
