Only one new form must be turned in this year. Proof of a recent vision screening. Click on the purple link below to see the form.
A new vision screening is required. The school gets audited after Winter break for compliance.
May be completed by an eye doctor OR Primary Care Provider
Exams must be performed no earlier than one year before enrollment or no later than 6 months after enrollment to be in compliance with IAC 641 Chapter 52
Bike Rodeo
Bike Rodeo
Each spring, a bike rodeo is planned for all 3rd graders.
The class will spend approximately 1 hour outside with Safe Route to School volunteers learning bike and pedestrian safety and practicing bike skills.
A letter will go home closer to the event with more details and will include a permission form to be signed and turned back into your child's teacher.
Your child may bring their bike or scooter and a helmet (required) to use the day off.
Other things you may want to pack for your child for that day: water bottle, sun screen, and a light jacket/sweatshirt.
They should wear closed-toe shoes that day.
