Must be completed no later than 1 year before and 4 months after enrollment
Must be provided by a licensed dentist or dental hygienist
Must use IHHS (IDPH) Certificate of Dental Screening form

Dental Screenings (no treatments) will be completed, before Winter Break, on all Freshman students who do not have an IHHS Dental Certificate on file required for enrollment.
Students without forms on file will automatically be screened unless the parent/guardian submits a note asking to exclude the student.
