No new forms needed this year. Unless forms are missing from Kindergarten or your child is enrolling for the first time (see KDG list) OR if your child is participating in sports--then a new Athletic Physical is required.
It is still recommended you schedule a yearly well child visit.
It is good practice to turn in a new signed Immunization Certificate every time your child receives a new shot.
If your child was up to date on their shots last year, they are good again this year.
If your child has an exemption form on file, these do not expire.
If your child is participating in sports: A new Sports Physical Form is required.
Must be completed no later than 1 year prior and 4 months after enrollment (of 9th grade)
Must be provided by a licensed dentist, dental hygienist
Must use IHHS (IDPH) Certificate of Dental Screening form
If a form is not turned in by the end of Sept. their Freshman year, your student will be put on a list to be screened by Public Health.
Dear Parents/Guardians of incoming Freshman:
There are many things on the to-do list as your child transitions to high school. In addition to getting coursework straightened out, the Health Office has a couple of requirements as well.
All children enrolled in Ninth Grade are required to have a dental screening pursuant to IDPH 641 Chapter 51.
A screening for ninth grade is valid from one year before enrollment to four months after enrollment date (August 2024).
It may be provided by a licensed dentist or dental hygienist only.
The Iowa HHS Certificate of Dental Screening is the only acceptable form.
Please turn in the paper copy to the high school office or have the paper faxed to the school by the office completing the screening.
Contact the District Nurse with any questions.
Sports Physical is required every year for all students participating in sporting activities.
All pages must be completed and turned in, not just the last page.
It must be completed and turned in before the sports season starts.
Contact your child’s coach with any questions.
Immunizations
If your child is up-to-date in 8th grade, there are no additional immunizations needed for Freshman year. Please check with your child’s provider to ensure that they are current. Your child’s record will move from middle school to high school at the end of the year. Exemption forms are accepted and do not expire.
Medications/Health Services in High School
If your child has scheduled or as-needed prescription medications that need to be taken at school, not much will change in high school.
Medications in High School need to follow all District Policies, which can be found on the District website and handbook.
We prefer the medication to be stored securely in the Health Office and administered by an adult.
In High School there are certain circumstances where a student may be able to carry their prescription medication on their person, but MUST be discussed with the School Nurse first and the appropriate paperwork completed.
If your child has a rescue medication (inhaler/epi-pen/glucagon pen/etc.), please get in touch with the District Nurse.
A new medication consent form needs to be signed by the parent/guardian each school year. There are different forms depending on the type of medication and who will be administering the medication. The nurse will help you determine which consent form is needed.
If your child has medical needs requiring management during the school day, please get in touch with the district nurse.
If there are health services listed in your child’s IEP or 504, these services will continue as before.

We understand and respect that your child is growing more independent; they are not adults yet. You may trust your child to have medication (prescription or over the counter) in their backpack or locker and think it’s ok no one at the school knows about it. However, we need to think of the safety and trustworthiness of all the students. Please have discussions about medication safety and responsibility with your child, whether they take medications or not. Safety is the top priority of ours for all of our students; this is why, if your child is carrying medication on their person, you notify the nurse, and the correct paperwork is completed.
