Required Parent Notice of Scholarship Programs for Students with Disabilities Senate Bill 316 established a new requirement related to students with disabilities that becomes effective on September 24, 2012. Per Ohio Revised Code Section 3323.052, each time a school district completes an evaluation for a child with a disability, or undertakes the development, review or revision of the child’s IEP the district shall notify the child’s parent by letter or electronic means about both the Autism Scholarship Program, under section 3310.41 of the Revised Code, and the Jon Peterson Special Needs Scholarship Program, under Sections 3310.51 to 3310.64 of the Revised Code. "When to Provide Prior Written Notice, Informed Consent, and Procedural Safeguards Notice (A Guide to Parent Rights in Special Education)"

The Decision Framework for Special Transportation is used to determine when a student with a disability should be provided with “specialized transportation”, meaning curb-to-curb service. This form will now be a required part of a student’s IEP if specialized transportation will be provided instead of the student using standard transportation options. This form should be completed in EdPlan when discussing transportation.

Notice of Eye Examination - Parent Letter

EYE EXAM REQUIREMENTS - Ohio law (H.B. 95) requires that each student newly identified as a child with a disability must have a comprehensive eye exam at private cost. According to the law, Cincinnati Public Schools has no obligation to pay for this exam, unless the exam is required as part of an evaluation or re-evaluation. The following letter must be given to parents of newly identified students. Reporting Eye Exams Senate Bill (S.B.) 316 continues the requirement for students with disabilities to undergo a comprehensive eye examination within three months of receiving special education services for the first time.
