At St. Elizabeth CHS we are dedicated to providing students with all of the support they need as they transition from high school into post-secondary education. Planning for this transition is something we begin preparing students for at the start of high school, and it is something we encourage throughout a student's high school journey. While Guidance Counsellors and Special Education teachers will work collaboratively to assist students with the process, students and parents are encouraged to begin exploring various post-secondary destinations prior to the start of Grade Twelve.
Parents/Guardians and students are encouraged to connect with post-secondary institutions Accessibility Offices in order to determine what special application requirements and/or documentation will be required in order to continue to receive support at the post-secondary level.
Given that each student’s needs are unique, services and accommodations are established on an individual basis and based on the documentation of the student’s disability.
Please note: Secondary school staff cannot share any information regarding students with colleges/universities. It is the responsibility of the student to provide all of the necessary information and documentation with post-secondary institutions.
Post-Secondary institutions will only become aware of accommodations and supports if the individual chooses to disclose them.
Educational institutions have a legal obligation to provide reasonable accommodations that promote equity for people who have disabilities. The Transition Resource Guide was created by the Regional Assessment and Resource Centre (RARC) as way for students with disabilities to arm themselves with the knowledge they need to access resources at college and university, and to make a successful transition from secondary to post-secondary.

Understanding Accessing Accommodations
Post-Secondary Institutions may require the following information:
Psycho-educational assessments prepared by a registered psychologist or psychological associate outlining the type of learning disability and suggesting accommodation required
Medical documentation, prepared by a doctor or licensed medical practitioner outlining the diagnosis and limitations related to the specific disability, with recommendations for accommodations
Individual Education Plan, outlining accommodations and modifications made each year during attendance at secondary school.
