Release of Information

The Release of Medical Information form gives the school permission to get in touch with your primary care physician and/or any specialists you list in the case of an emergency, or if we need a note from a recent visit, lab results, return to class/sports notes, accommodations for class, etc. This is not a required form, but it is a form that makes it easier to get the information we need in a timely manner.
