This form is required if you would like your student to have permission to take non-prescription medication that is not typically available to our students with standard orders by Dr. Hall. Some of these medications may include:

• Lactaid

• Excedrin Migraine

• Claritin or Zyrtec

• Pepto Bismol

Some of these may need further clarification from a parent or guardian before the medications are administered.  Similar to prescription medications, over the counter medications need to be dropped off by a parent/guardian for a count/signature and receipt.
