Students that have been diagnosed with asthma often have something called an Action Plan. This is a form filled out by a physician/provider that covers the type(s) of asthma the student has, any potential triggers, as well as how to proceed in treating an asthma attack depending on severity/symptoms. This form is typically provided by a student's doctor, however we can always give you a copy if you need one.
Again, if your student has asthma that requires the use of an inhaler, and you want your student to be able to carry their inhaler with them, the Emergency Medication Self Carry Form, as well as the Prescription Medication Form needs to be filled out by a parent/guardian as well as the prescribing physician.
