Emergency Medication Self Carry

The Emergency Medication Self Carry Form is required for students who have life-threatening allergies or asthma who need to carry an Epi Pen or rescue inhaler on their person.  This form needs to be signed by the prescribing provider as well as a parent/guardian.  In addition, once this form is returned, the student needs to meet briefly with the nurse to demonstrate their ability to properly carry, store and use their medications.  We also need the students to understand the level of responsibility that comes with carrying medication.
