This page will connect you with all the health forms that are REQUIED FOR ALL OF OUR STUDENTS. In addition, this page will link you to many of the other forms you might need depending on your student's individual health needs.
Each student is REQUIRED to have the MEDICAL INFORMATION FORM
completed and updated every year.
We really encourage everyone to also fill out the MEDICAL RELEASE FORM, which gives us permission at the school to send forms back and forth to physicians and other health care providers, as well as to contact providers in the event of an emergency. Finally, please fill out the form with State Required Health questions - it is a survey that the state of Vermont conducts annually to assess the general health and wellness of our school age population.
