Postpartum HTN Telehealth Program Order Set and Program (Updated 4/4/22)

Information about STAC (Staying Healthy After Childbirth) program at Meriter

Epic Order Referral – (Tip sheet) Epic Order: “Ambulatory Referral to PostPartum Telehealth-Meriter”(REF705) will be entered by the OB Provider for the patient meeting home monitoring program criteria. Telehealth staff will utilize an Epic work-que to enroll patients. The work-que is generated from the order referral. Therefore, OB Providers must enter the following in the order referral:

  1. Is the patient aware of the referral?

  2. Actual Delivery Date

  3. Expected Discharge Date

If needed, the RN may enter the order referral and would be responsible for completing the above information.


Enrollment Process

  1. Provider enters the order referral in Epic

  2. Charge RN on postpartum will review the Epic work-que list day before 0900 - put in order before this time

  3. Telehealth MA views Epic work-que and patient list report to see who has a referral order and needs to be enrolled that day

  4. Telehealth MA will Vocera the charge RN at the start of each shift (if charge nurse is unavailable the MA will be patient rounds/enrolling)

  5. Charge RN confirms patient priority needs with the MA, if applicable; e.g. patient wants to discharge a.s.a.p.

  6. MA begins rounding / enrolling patients

Cancellation Process – If the patient declines participation in the program, the following will occur:

  1. MA will tell the patient’s RN the patient declined enroll in the program

  2. RN will call the patient’s provider to discuss a new plan

  3. MA will cancel the Epic order referral

  4. MA will write a progress note in the patient’s chart and document the reason the patient decided not to enroll
