Special Dietary Needs
When a student has been identified as having a medical condition and/or a disability that requires special dietary accommodations, a written medical statement from a licensed healthcare provider is required before any changes can be made to school meals. This applies to:
Eliminating specific food items
Making any other modifications to the school-provided breakfast or lunch
Please note:
Diet orders do not expire, but a new written statement is required for any changes, including stopping the diet modification.
All dietary accommodations must comply with federal school nutrition guidelines and are coordinated through School Nutrition Services.
Special Diet Needs: This form must be completed by parent/guardian AND medical provider to allow the nutrition department to provide unique meal substitutions.
Milk Substitution: This form can be completed by a parent/guardian and will allow the nutrition department to provide fluid milk substitution only.
