Immunizations

As of 2025, please be aware of North Carolina’s immunization and health assessment requirements for certain students. If records are not submitted by the 30-day deadline, students will NOT be allowed to attend school until documentation is received, per state law.

Who needs updated immunization records?

Kindergartners
7th graders
12th graders
Students attending an NC public school for the first time

(This includes students enrolled in Wilson Academy of Virtual Education)

Deadline: Records must be submitted within 30 calendar days of the first day of school:

Early Colleges first day of school: August 4
Traditional Schools' first day of school: August 25

Health Assessments Required:

For all kindergartners and first-time NC public school students
Must be completed by a medical provider
Accepted if dated within one year before the first day of school

COVID-19 vaccines are NOT part of the required immunizations.

Kindergarten Immunization Requirements

- The North Carolina Immunization Law requires all children entering Kindergarten or entering North Carolina Public Schools for the first time to receive the following vaccines before starting school: DTP (5 doses), Polio (4 doses), MMR (2 doses), Hepatitis B (3 doses), Varicella/Chickenpox (2 doses).
- North Carolina Law also requires all children entering Kindergarten or entering North Carolina Public Schools for the first time to provide the school with a completed NC Health Assessment/Physical Form, which must be completed by a health care provider within the previous 12 months of their first day of school. - Students are allowed thirty calendar days from their first date of attendance to become compliant with immunization and health assessment requirements. Principals must exclude any student not in compliance with this law upon the end of the thirtieth calendar day.

7th Grade Immunization Requirements

- The North Carolina Immunization Law requires all children entering 7th grade to receive the following two vaccines:
  - Tdap Vaccine: One booster dose of Tdap vaccine for individuals who have not previously received it and are entering 7th grade or by 12 years of age, whichever comes first.
  - Meningococcal Vaccine: One dose for individuals entering 7th grade or by 12 years of age, whichever comes first.

12th Grade Immunization Requirements

- Meningococcal Vaccine: One booster dose for students entering 12th grade or by 17 years of age, whichever comes first. Individuals who receive their first dose of meningococcal vaccine on or after the age of 16 do not need a booster dose.
- Students are allowed thirty calendar days from their first date of attendance to become compliant with immunization requirements. Principals must exclude any student not in compliance with this law upon the end of the thirtieth calendar day.
