Chronic Health Conditions
At the beginning of each school year, and when a student enrolls for the first time, parents/guardians are asked to complete and/or update the health history of their child to help us understand and plan for any medical needs.
Our goal is to ensure that health concerns never become a barrier to learning. When a student has a diagnosed chronic health condition (such as asthma, diabetes, seizures, or severe allergies), and the parent or guardian notifies the school, the school nurse will develop an Individual Health Care Plan (IHP) or Emergency Action Plan (EAP), as needed.
This plan outlines how the student’s health needs will be supported during the school day and will be shared with appropriate school staff to ensure a safe and supportive learning environment.
If your child has a chronic health condition, please notify your school nurse as soon as possible so we can work together to support their success.
