A one-time visit provided to follow up with mother and baby after hospital discharge.
Postpartum/Newborn home visiting services are provided at no charge by Health Department nurses and to all Medicaid eligible women and/or their infants shortly after birth.

Verify and assist with arranging medical appointments
Early identification of any health or safety concerns
