Fix USB - Format Failed

When you try to format a USB memory stick you get a message saying "The disk is write protected"

This also happens when you do this via

My Computer (right-click)

Manage->Disk Management

Select your USB device and try and format: You get a message saying

"The operation did not complete because the media is write protected".
