Comply with the dependent overseas screening procedures provided to you by your detaching command and/or clinic/hospital.
The steps below only serve as an example of what the process may be similar to. Adhere to the actual instructions provided to you by your detaching command and clinic/hospital.

PHA will most likely ask for a copy of your orders more than once, ALWAYS bring a copy with you.
Pickup Overseas Screening packet from PHA window at clinic/hospital. Complete any sections of the paperwork which you are instructed to complete.
Go to Immunizations with your shot record for review.
If you don't have your shot records with you, they will simply have to draw blood to preform a titer test in order to make sure your shots/vaccines are up to date.
Once they draw blood, the results will be back within 48 hours.
You will have to call the Clinic to find out what shots you need, if any and get them done before they can sign off on the Overseas Screening Packet.
Females, 21 and up, must have a current Well Woman's Exam (pap smear) (within the last 3 years).
If you are up to date, bring your OBGYN records for proof and they will sign off.
If not up to date, make an appointment at the NS Mayport Health Clinic or get a referral to a doctor in town.
If you go out in town, bring the records of the exam/results to the Clinic for them to sign off.
Retrieve copy of most recent dental x-ray and dental exam and bring to military dental clinic for review and signature.
If you don't have your dental records, you will need to make an appointment for x-rays and an exam for them to sign off on the paperwork.
Return to PHA window to make appointment with doctor for screening packet review and signature.
When dependent screenings are complete (after step 5 is complete), member will complete Part I of NAVPERS 1300/16 to include Interviewer name and signature (member's supervisor).
Bring completed screenings and NAVPERS 1300/16 to Overseas Screening office (PHA) at clinic/hospital for review and signatures.
Make sure an O-4 or higher, or civilian medical doctor, is the person that reviews and signs the Overseas Screening Packet.
Member then routes completed screenings including NAVPERS 1300/16 up to member's CO.
