NO FEE PASSPORT

***It is essential to start the No-Fee Passport process sooner rather than later. It takes PSD about a month to a month and a half to obtain the No Fee Passport and get it back to you***

***THE NO-FEE PASSPORT ACTS AS YOUR VISA IN SPAIN***

• ALL DOCUMENTS ARE ATTACHED BELOW.

• Review the No-Fee passport check list before setting an appointment with PSD.

• Also, please follow the instructions EXACTLY as listed on the No-Fee DS-11 online form and No-Fee DD 1056 documents.

• There are also instructions on how to apply for passport for a minor, please follow them EXACTLY, both parents must be present for minor children. If either biological parent can't be present, they will complete the DS-3053 and submit with copy ID both sides.

• Lastly, once you complete DD Form 1056 (attached), please bring your form to Admin to receive signature from Authorizing Official.

1. Get a passport photo taken at one of the locations listed below. Requirements are also listed below.

2. Fill out No-Fee DS-11 online form and the No-Fee DD 1056 documents.

   1. DS-11 MUST BE TYPED ONLINE. NO HANDWRITTEN APPLICATIONS WILL BE ACCEPTED.

3. Make an appointment with NS Mayport PSD (or YOUR local PSD).

   1. Bring all No-Fee documents, a copy of your birth certificate, current passport (if you have one), a copy of your DoD ID, copy of orders, and the passport photo.

4. About a month later, PSD will reach out to inform you that your No-FEE passport is ready for pick up.

Passport Photo Location Recommendations:

• Walmart

• CVS

• Walgreens

Requirements for Passport Photo:

• Submit a color photo, taken in last 6 months 2.Use a clear image of your face. Do not use filters commonly used on social media.

• Have someone else take your photo. No selfies.

• Take off your eyeglasses for your photo.

• Use a plain white or off-white background

Resolution, Print Size, and Quality

• Submit a high resolution photo that is not blurry, grainy, or pixelated.

• The correct size of a passport photo is: 2 x 2 inches (51 x 51 mm) *Head must be between 1 -1 3/8 inches (25 - 35 mm) from the bottom of the chin to the top of the head *Printed on matte or glossy photo quality paper *Do not digitally change the photo *You cannot submit a damaged photo with holes, creases, or smudges.

• Taken in clothing normally worn on a daily basis.

• You cannot wear a uniform, clothing that looks like a uniform, or camouflage attire.
