Grading System and DepEd School Forms
This activity enabled us to attain the following intended learning outcomes: 1. identify the school forms, its purpose and uses; 2. download and fill-out sample DepEd school forms; and 3. do sample computation of grades using the DepEd assessment criteria.
This activity focuses on grading system and DepEd school forms where we experienced computing grades using the format used by DepEd in the present. We also familiarized with the different school forms by accomplishing samples of SF 1,2,3,5,8,9 and 10.
In this activity, we answered all the parts namely: the observation, analysis, create, and reflection. In the observation part, we answered the table which requires computing the grades of students base on the previous lesson in our assessment in learning subject. The analysis part allowed us to identify the different approach of computing grades of the students and what is the purpose of it. We also identified the different school forms accomplished by school advisers. We also created and provided sample data of the students and input it in the different school forms. Lastly in the reflection part, we identified what kind of approach we prefer to use in computing the grades of the students, through manual or through spreadsheet where it automatically computes the result.
In this activity, we learned the value of providing accurate information of students in the different school forms and the importance of making these forms available on the exact time where it is needed.

The Activity above was anchored on the fourth and fifth domain, Curriculum Planning and Assessment and Reporting. Upon answering the activity, we were able to achieve different learning competencies that we are expected to attain upon completing it.
The observation part helped us attain the 5 learning competencies: (1) Show skills in the selection, development and use of a variety of teaching and learning resources, including ICT, to address learning goals, (2) Demonstrate knowledge of monitoring and evaluation of learner progress and achievement using learner attainment data, (3) Demonstrate knowledge of providing timely, accurate and constructive feedback to improve learner performance, (4) Demonstrate familiarity with a range of strategies for communicating learner needs, progress and achievement, and (5) Demonstrate an understanding of the role of assessment data as feedback in teaching and learning practices and programs.
This part of the activity helped us attain the following learning competencies: (1) Demonstrate knowledge of the design, selection, organization and use of diagnostic, formative and summative assessment strategies consistent with curriculum requirements, (2) Demonstrate knowledge of monitoring and evaluation of learner progress and achievement using learner attainment data, (3) Demonstrate knowledge of providing timely, accurate and constructive feedback to improve learner performance, (4) Demonstrate familiarity with a range of strategies for communicating learner needs, progress and achievement, and (5) Demonstrate an understanding of the role of assessment data as feedback in teaching and learning practices and programs.
The analysis part of this activity helped us attain the following learning competencies: (1) Demonstrate knowledge of the design, selection, organization and use of diagnostic, formative and summative assessment strategies consistent with curriculum requirements, (2) Demonstrate knowledge of monitoring and evaluation of learner progress and achievement using learner attainment data, (3) Demonstrate knowledge of providing timely, accurate and constructive feedback to improve learner performance, (4) Demonstrate familiarity with a range of strategies for communicating learner needs, progress and achievement, and (5) Demonstrate an understanding of the role of assessment data as feedback in teaching and learning practices and programs. These are under domain number 5. This activity also allowed us to attain different learning competencies under domain number 4 and these are : (1) Prepare developmentally sequenced teaching and learning process to meet curriculum requirements, (2) Demonstrate knowledge in the implementation of relevant and responsive learning programs, (3) Seek advice concerning strategies that can enrich teaching practice and (4) Show skills in the selection, development and use of a variety of teaching and learning resources, including ICT, to address learning goals.
