An interprofessional team's vision and knowledge sharing is needed to understand the client's holistic approach and identify person`s health literacy skills.
In a multidisciplinary team, it is important to share the knowledge of each professional and to listen to the voice of the client in order to achieve a common vision and shared goal.
Collaboration is built on appreciation between the interprofessional team members, respectful communication, shared dialogue and joint purpose. Professionals need to recognize the individual needs of the client, for example health literacy skills.
The ICF framework, from which the Interprofessional person-centered assessment form has been modified by Snyman 2016, is used as a reference framework for understanding the client's functional capacity.
The rehabilitation goal is set together with the client and professionals. Principles of goal-setting with Goal Attainment Scale (GAS) are specific, measurable, achievable, realistic and time-bound.
