County Health Rankings offers excellent data aggregation and access to state data by county, using the social determinants of health as their framework. Metrics of Colorado Counties, from County Health Rankings, have been downloaded to build a pivot table with features for selecting the data you need or want. See examples of extracts from the pivot table. Download the pivot table to open in Excel; then select other counties and "save as" a pdf file. Stakeholders should include this data in their "Community Health Assessments". Without data transparency, no accountability.

County Health Rankings provides us with data taken from many different data sites. They provide insight to the Social Determinants of Health. The website is informative in many ways, and it is our hope these files below will complement the work CHR is doing. Download the Excel files to explore the pivot tables and select your county to see not just the metrics but a context of quintiles as well as a Health Equity Index. The variation between counties is stunning!
Similar metrics are available at the city level, 500 cities, the City Health Dashboard. We are waiting for the data set to be published.
