Family Medicine Patients only will have more detailed information in their online chart

• Please go to this site and type all of this into the search bar to access your Athena Health Portal: https://24994-1.portal.athenahealth.com/

• instructions were likely given to you at your first visit (as long as we have a valid email on file)

• you will need your login information (username and password)

• please ask for new access information if you are having a problem (we will need your correct valid email address on file)

• Since this is a third party software, our staff may not be able to help with technologic issues accessing your Athena patient account.
