Complete: this measure is being implemented, data collection is in place, and CCHD has met the 2023 year-end target for this measure in the current or a previous quarter. (Action Taken by Department = Maintain progress in next quarter).

On Track: this measure is being implemented, data collection is in place, and quarterly data indicates CCHD is on track to meet the year-end target for this measure by the end of 2023. (Action Taken by Department = Maintain progress in next quarter).

In Process: this measure is being implemented, data collection is in place, and CCHD has not met the 2023 year-end target for this measure. However, quarterly data does not necessarily indicate that CCHD will not meet the target for the measure by the end of 2023. (Action Taken by Department = Re-assess progress in next quarter).

Not on Track: this measure is being implemented, data collection is in place, and quarterly data indicates CCHD is currently not on track to meet the year-end target by the end of 2023. (Action Taken by Department = Create plan of action to improve progress in next quarter, should be primary departmental focus).
