SECURING A LIBRARY CARD

A library card is required for borrowing library materials. It is also required in any library transactions. Use of one’s library card is strictly prohibited. Lost library card should be reported immediately to the Librarian.

For new student

• 1. Two (2) copies 1x1 recent picture (colored)

  2. Enrollment Print-out/Valid School ID

For lost library card

• 1. One (1) copy 1x1 recent picture (colored)

  2. Affidavit of Loss
