The Division of Vocational Rehabilitation (DVR) shall coordinate with officials of the Colorado Department of Education (CDE) and local school districts around development and application of policies and practices with regard to the planning and provision of transition services to students and youth with disabilities. DVR and CDE shall maintain a state-level formal CDE/DVR Cooperative Services Agreement related to the planning and provision of transition services. This agreement shall include: The roles and responsibilities of each agency (including financial responsibilities), the provisions for determining state-led agencies and qualified personnel responsible for transition services, and the procedures for outreach to and identification of students and youth with disabilities.
State agencies, education partners, families, and service providers have collaborated to develop a Sequencing of Services framework, which describes a shared vision and shared outcomes for students with disabilities during their transition before and after high school. The Sequencing of Services framework demonstrates the programming and services, across a continuum of ages, that are available to students and how different programs can all work together to ensure student transition needs are met.
