When the application has completed the application and initial survey process with CDPHE, and has completed the Provider Enrollment process with Health Care Policy and Financing (HCPF), HCPF will be the entity to issue final approval. Final approval will be sent to the provider, CDPHE, and all Case Management Agencies selected by the provider. When the Program Approval Form is received by CDPHE, the approved agency will be added to this list.
