This page will guide you through how to submit changes of information to both CDPHE and HCPF. All changes must be submitted separately to BOTH entities, with the exception of changes to your Case Management Agencies.
There are two way to notify CDPHE of closure. All agencies voluntarily closing or terminating all PASA services should notify both CDPHE and HCPF of this. The process is outlined on this page.
