If your agency is closing, or will no longer be providing PASA services, you will need to notify both CDPHE and HCPF of this.
For CDPHE purposes, this can be done in one of two ways:
Option 1:
Follow the instructions on the Changing/Updating Your Information page, selecting the below option on page two of the form, after identifing your Facility/Agency
Option 2:
Send an email directly to CDPHE HFEMSD at CDPHE_PASA@state.co.us including the following information
Name of Agency
Summary of reason for closure/termination
Effective date of closure/termination
We will follow up with you to confirm closure, and to request additional information if needed.
Providers who are choosing to close or terminate all PASA services will also need to disenroll with Health Care Policy and Financing. Instructions for this are linked for you here:
