The Colorado Department of Public Health and Environment (CDPHE) PASA application is only one of two required applications in the process to become a Program Approved Service Agency. Prospective providers must ALSO submit a Medicaid Provider Enrollment Application through Health Care Policy and Financing's (HCPF) Provider Web Portal. CDPHE does not have access to this web portal, so questions about this process must be sent to HCPF through their Fiscal Agent. In addition, the websites linked below belong to HCPF, not CDPHE. They are provided here for informational purposes and guidance.
You will not be approved to bill for Medicaid services until both CDPHE and HCPF have reviewed and approved your applications.
HCPF has the final approval authority, but will require CDPHE's recommendation for approval prior to taking final approval action.

Evidence of this HCBS provider enrollment training should be uploaded with other supporting documents when submitting the HCPF Provider Enrollment Application. This does NOT need to be submitted as a part of your CDPHE PASA Application.
