General Health Facility Questions:

Time Expired to Create Account:
If the link you received via email to create your account in COHFI has expired, click the "Forgot/Reset Password" button and enter the email associated with your account. If an account exists for that email address, a link will be sent where you can reset the password for the account. Note: we cannot change your password for you, you must use the forgot/reset password option.
Forgot Password:
If you forget your password, or it has expired, please click the "Forgot/Reset Password" button. Enter the email address associated with your COHFI Account, and an email will be sent with a link that will allow you to reset the password.
Sharing COHFI Accounts is Prohibited:
Each person accessing the COHFI portal must have their own account. Your COHFI Account Manager can assign users as needed. Contact your facility's COHFI account manager for access, as we cannot assign users to your account.
COHFI Account Manager:
Only CDPHE has rights to assign the COHFI Account Manager. If the COHFI Account Manager needs to be changed, please contact cohfi.support@state.co.us for assistance. The COHFI Account Manager role has the ability to create new users, assign existing users to a facility, remove existing users from a facility, and assign rights to users assigned to each facility.
Facility Account Users:
These user accounts are created by the COHFI Account Manager when additional individuals require access to the system. These accounts are able to be assigned as COHFI Account Managers by CDPHE. Each user can be assigned to multiple facilities/agencies in COHFI. Users should NOT create separate accounts for separate facilities.
Finding Assigned Users & Rights:
The COHFI Account Manager will have the Manage Users button that will allow them to add and remove users, and assign rights after they have selected the facility. All external user accounts can navigate to the Facility menu and select Facility Users to see all individuals assigned to each facility, and the rights each user has.
User is Locked Out:
The COHFI system will automatically lock user account that have not logged in for 90 days. If you are locked out, please contact cohfi.support@state.co.us for assistance
User is Not Active:
If a user does not have any facilities assigned to them in COHFI, their account will be inactivated when they attempt to log in.
