Additional Required Documents for the Initial PASA Application

The initial PASA application that we provide for you to complete is NOT the only document required to be submitted. Not all items from this list will apply to all applicants, but failure to submit required documents for the services your agency is applying for will delay our ability to process your application.

The online application form will guide you through what additional documents may need to be submitted, and will allow you to submit those documents within the application itself. These additional documents must be provided to CDPHE at the time of application in order for us to review your application.
