PASA Provider Qualification Requirements

Each service/specialty under the PASA program has it's own set of qualifications a provider must meet in order to be a provider of that service/specialty. 

If your agency is applying for a service/specialty, you must provide an organizational chart showing the qualified staff person, as well as the license/certification or other qualifying document, along with a resume detailing that individual's experience and how it related to the services/specialties being applied for.
