COVID-19 Testing Consent 

I understand that Juan Diego Catholic High School requires signed consent for COVID-19 testing.

I understand that I need to sign it before my student can be tested. 

I understand that this consent form will be valid through June 2024, unless I notify the school in writing that I revoke my consent. 

I understand that if I revoke my consent or refuse to sign, that would preclude my student from participating in school or athletic or extracurricular activities sponsored by the school at the school's discretion. 

I understand that my student’s test results and other information may be disclosed as required by law.

I give consent to Juan Diego Catholic High School and its staff and/or contractors to examine and test my student by signing this Student & Parent Handbook.

I attest that I authorize Juan Diego Catholic High School (including school nurses, school nurses, physicians, or other medical staff, contractors, and school staff) to test my child for COVID-19. By signing this handbook electronically, I am attesting that I have the requisite legal authority and power to make the decisions for, and on behalf of my student.
