Standard 4: Comprehensively conducts assessments

Correct nursing assessment and history taking is always an important aspect of disease diagnosis, identification of the deteriorating patient, and making necessary referrals. I have always demonstrated comprehensive assessment whether it is system-related or general assessment which is proved through my clinical placement book procedure achievement summary. For example, I understand the importance of correct vital assessment and pain assessment. I always used the comprehensive PQRST pain assessment tool whenever patients said they are in pain. The assessment result provides the detail about the pain and give insight on the final diagnosis and interventions.
