My idea (2010-06-06 1933h) is for a modular system of a comfortable waste strap with belt-loops that goes on under your trousers. These are great because They're clandestine, harder to pickpocket even if the thief knows that they're there, and they offer you about the same access ability as a pocket does.

Neck-Attached stuff doesn't do you any good if the thief can see the strap; I had a Palm Centro on a Lanyard under my shirt that was still stolen on a micro-bus in Nairobi.
