Data Organization Page: During the 2012-2013 school year our district began working with a new math program that included a variety of assessments. Students take a pretest before each topic, complete a quick check or formative assessment following each lesson, do a topic test to end the topic, and complete a bechmark assessment every four topics. These multiple assessments have proven to be very valuable to my instruction. I have attached a form I created and have used as a way to organize the data from these assessments in order to guide my planning and instruction. Each form includes space for me to record student information based on the pretest, space to plan groupings for differentiated instruction during the lesson in response to that pretest data, space to record student scores on our formative assessment, and space to note plans for the next day's math workshop time in response to formative assessment data.
